Terms and Conditions for The Med Chat $250 Gift Card Offer
1. Eligibility: This offer is only valid for new patients who have no prior patient/physician relationship with the plastic surgeon or practice. A “new patient” is defined as someone who has not previously established any patient/physician relationship with the practice.
2. Engagement Requirement: To qualify for the $250 gift card, individuals must engage with The Med Chat Coordinators and utilize The Med Chat Platform to manage their surgery booking and correspondence.
3. Financial Disclosure: Eligible patients must agree to share financial information related to the completion of the procedure. No protected health information is required to be disclosed under this offer. Should patients choose to share personal health information, they must sign a separate consent form.
4.Testimonial Requirement: As part of this offer, patients are required to provide a testimonial about their experience with The Med Chat on at least one platform. The Med Chat agrees to allow patients to maintain the level of privacy they are comfortable with, provided the testimonial does not reveal details that could identify the patient.
5. Gift Card Distribution: The $250 gift card is provided directly to the practice and will be issued within 24 hours of the completion of the surgical procedure.
6. Minimum Procedure Cost: The offer is only valid toward surgical procedures with a minimum surgeon fee of $5000.
7. Surgeon Requirements: Patients are free to select any plastic surgeon, provided that the surgeon is board certified and in good standing within the United States.
8. Discretionary Rights: The Med Chat reserves the right to deny services, including the issuance of the $250 gift card, solely at their discretion.
9.Non-Monetary Value: This offer has no monetary value and cannot be exchanged, transferred, or redeemed for cash. There are no guarantees implied beyond the provision of the $250 gift card upon completion of the terms stated herein.
